Insurance reimbursement

We are in network with the insurance companies depicted below. If you do not see your insurance company listed you are out-of network with gabel PT + wellness. We will provide an itemized super bill and receipt so you can submit to your insurance on your own for reimbursement. 

Montana is a direct access state. This means that legally a physical therapist can see you without a doctor referral. With that being said some insurance companies will not provide payment unless you have a referral from a physician. As a courtesy, gabel PT + wellness will check your benefits and let you know if you need to obtain a referral prior to your initial visit. We strongly encourage you to know your own PT benefits as we are commonly misquoted.

You will be accountable for your copay or coinsurance at the start of each session.
