What to Expect on Your First Visit

The first visit consists of a highly skilled evaluation including a detailed subjective history and hands on objective assessment.  The initial visit is 60 minutes long. Please wear comfortable loose fitting clothing suitable for movement. Treatment will almost always be provided on your first day. The overall plan of care, goals, and time frame will be discussed at the end of your visit. We are really looking forward to meeting and working with you.  

Intake forms

To maximize evaluation time on your first visit please fill out forms prior to your visit. This will be sent to you electronically prior to your visit. If you choose to fill the forms out at the clinic please arrive 15 minutes early to your appointment. The HIPAA policy is also below for you to review. Please click on HIPAA to open.

If you must cancel your appointment gabel PT + wellness requires 24 hour notice. Please email gabelpt777@gmail.com or call 406.589.6083 prior to your appointment. Same day cancellations will be charged a fee of $150. If you do not show up for an appointment you will be charged the entire cost of the visit.
